The Lakefront Dry Eye Protocol: A Complete Plan for Lasting Relief

This isn't a one-and-done treatment. It's a comprehensive clinical pathway that combines advanced in-office therapy with daily at-home care — because lasting relief requires both.


What Is MGD?

Meibomian gland dysfunction is the leading cause of dry eye disease. Your meibomian glands — tiny oil-producing glands in your upper and lower eyelids — are responsible for creating the lipid (oil) layer of your tear film. When these glands become blocked, inflamed, or atrophied, your tears evaporate too quickly, leaving your eyes dry, irritated, and uncomfortable.

The problem: MGD is progressive. Without treatment, gland blockage leads to inflammation, which leads to gland atrophy (permanent structural loss). The earlier you intervene, the more gland function you preserve — but even patients with significant gland loss can benefit from treatment.


How Our Protocol Works

Step 1: Comprehensive Eye Exam + Tear & Gland Evaluation

Everything starts with a thorough assessment:

  • Meibography (Firefly Digital Slit Lamp) — infrared imaging to visualize the structure of your meibomian glands and assess how much healthy tissue remains

  • Tear film analysis — evaluating tear quality, stability, and evaporation rate

  • Ocular surface evaluation — assessing inflammation, lid health, blink patterns, and contributing factors

  • Tear and gland evaluation with Jessie — our medical aesthetician performs a detailed gland assessment as part of your treatment planning

This data tells us exactly where you stand — from mild gland congestion to severe atrophy — and allows us to tailor the protocol to your specific condition.


Step 2: Diagnosis & Treatment Planning

Based on your evaluation, Dr. Bert or Dr. Kucha will diagnose the type and severity of your MGD and create your personalized treatment plan. Most patients with MGD benefit from both the in-office protocol and daily at-home therapy working together.


Step 3: At-Home Therapy — The 4 H's

Your at-home routine is the foundation of your treatment. We call it the 4 H's — and consistency is the key:

🔥 HEAT — Warm Compresses
Thickened, congested eyelid glands need warmth to liquefy blocked oils. Warm compresses soften the material clogging your glands so they can begin releasing healthy oils again.

  • Perform 1–2 times daily (depending on severity)
  • At least 5 minutes per session
  • Recommended: D.E.R.M. Mask, Bruder Mask, or Wizard USB Heated Mask


🧼 HYGIENE — Lid & Lash Care
Just like dental plaque forms on teeth, your eyelids harbor a mixture of beneficial and harmful bacteria. Overgrowth of harmful bacteria leads to MGD, blepharitis, and even demodex mite infestation. Daily lid hygiene keeps bacterial populations in check.

  • Clean lids and lashes daily
  • Recommended: Nova Hypochlorous Acid Spray, Avenova, Zocushield, Zocufoam, or Zocuwipes

💊 HEALTH — Omega-3 Fatty Acids
Your meibomian glands need omega fatty acids to produce healthy oils. High-dose triglyceride-form Omega-3s also have powerful natural anti-inflammatory properties specifically formulated for MGD and dry eye relief. Not all omegas are created equally — we recommend clinical-grade supplements.

  • Recommended: PRN Dry Eye Omega, HydroEye, EyePromise, or Nordic Naturals


💧 HYDRATE — Preservative-Free Artificial Tears
If your eyes aren't producing quality tears, preservative-free artificial tears provide lubrication, calm inflammation, and flush toxins from the ocular surface. Preservative-free formulations avoid the irritation and toxicity that preserved drops can cause with regular use.

  • Use as needed throughout the day
  • Recommended: iVizia PF, Oasis PF, or Optase PF
  • For nighttime: EyeSeals or EyeEco moisture goggles with lubricating ointment

And don't forget to drink plenty of water — consider adding electrolytes!


Step 4: In-Office Toyos Protocol — 4 Treatment Sessions

The clinical centerpiece of our Dry Eye Protocol is a series of 4 treatment sessions using our Luvo Darwin™ system, following the evidence-based Toyos protocol:

What happens during each session:

  • IPL (all 4 visits): Intense pulsed light is applied to the full face, with the Toyos protocol specifically targeting the lower lids. IPL incinerates the abnormal blood vessels feeding inflammation around your meibomian glands — addressing the root cause of gland dysfunction, not just the symptoms. As a bonus, IPL also clears sun damage, reduces rosacea, and evens skin tone.

  • RF (visits 2 and 4): Radiofrequency energy gently warms the tissue around your glands, softening blocked oils and making expression more effective. RF also stimulates collagen production — tightening and rejuvenating the delicate skin around your eyes.

  • Gland Expression (visits 2 and 4): After RF has warmed and softened the blocked material, Jessie performs meibomian gland expression — manually clearing the glands to restore oil flow.

Step 5: Maintenance & Ongoing Care

After completing the 4-session protocol:

  • Continue your daily at-home 4 H's routine — this is what sustains your results
  • Periodic maintenance IPL/RF sessions may be recommended based on your response
  • Follow-up meibography to track gland health over time
  • Adjustments to your home care regimen as needed


Why This Protocol Works

Most dry eye treatments only address symptoms. Eye drops lubricate, but they don't fix the underlying gland dysfunction. Our protocol works because it targets the actual disease process:

  1. IPL eliminates the inflammation feeding gland dysfunction

  2. RF unclogs the glands so they can produce healthy oils again

  3. Gland expression clears the backlog of hardened material

  4. At-home therapy maintains gland health between and after treatments

This combination — treating the cause (inflammation), restoring the function (gland expression), and maintaining the health (daily 4 H's) — is why patients see lasting improvement, not just temporary relief.

Early intervention produces the best results. When we catch MGD before significant gland atrophy, we can prevent permanent gland loss and restore function more completely.

But even severe cases respond. Patients with significant gland atrophy still see meaningful improvement in symptoms and tear quality. The remaining healthy glands can often compensate when they're functioning properly — and reducing inflammation benefits your comfort regardless of gland status.
